Community Outreach Services provides social work resources to adult residents of Bloomfield. The goal is to link people with the services and programs that they need. Social Workers are available to meet with the resident and/or their family in their home or at the office. Telephone consultation is also available.
Information and assessment are provided for the following types of issues:
- Housing options
- Financial assistance
- Advocacy
- Transportation
- Home health care
- Child and adult daycare
- Assistance with health insurance issues
- Long-term care plans
Social Workers can also assist with the completion of housing and financial program applications. Follow-up and case management services are available. The staff works closely with family members and caregivers, the Police Department, the Housing Authority, the Health Department, other Town departments, and State and community agencies in order to provide the best possible service to Bloomfield residents.
