Certified Birth Records

The Town Clerk’s Office issues certified copies of birth records for individuals born in Connecticut. Certified copies may only be provided to eligible parties as defined by Connecticut law. The fee is $20.00 per certified copy, payable by cash or check.

Who May Request a Certified Birth Record:

  • It’s you and you are 18 years of age or older, or an emancipated minor or a certified homeless youth
  • You are the parent, legal guardian, legal custodian, grandparent, spouse, adult child or adult grandchild
  • You are an attorney-at-law representing the person named on the birth certificate or an eligible relative (see above), an agent of approved state or federal agency, a member of a Connecticut genealogical society (see the Connecticut State Library for more information) or a Conservator of Person

How to Request a Certified Birth Record:

To request a certified birth record, you may submit your request in person or by mail you will need:

  • A completed Birth Record Request Form

  • Valid government-issued photo identification

  • Payment of $20.00 per certified copy (cash or check)
